Passport Services

Adult Passport Application

You may submit a maximum of two passport application under one appointment slot.

The passport application form can be completed online from the APO on www.passports.gov.au
Print the form and submit it to the Embassy. The requirements for an Adult Passport Renewal are:

• The completed passport renewal application form

• Your current original passport.

• Another current and photo ID, such as your KITAS/Driver's Licence as proof of current residence.

• Two (2) photos (45 mm x 35 mm, chin to crown measurement should be between 32-36mm with white/light blue background) are also required.

• Processing time is at least six weeks after all requirements have been fulfilled correctly & payment received. Please note that priority processing is not available for applications lodged in Indonesia.

Passport validity is 10 years.

Child Passport Application (0 to 15)

You may submit a maximum of two passport application under one appointment slot.

Please be advised that for child passport renewal, one of the parents must lodge the application in person at the Embassy. If the child is 16 or 17 years old, she/he will need to attend the lodgement otherwise it is not necessary for the child to come to the Embassy. In order to lodge a passport renewal for your child, the following documents will be required to be brought at the time of lodgement:

• Completed form (Overseas Passport Application Form/PC8). The form can be completed on-line at www.passports.gov.au. On the main page please see under CHILD PASSPORT INFORMATION and click "Applying for a Child passport while overseas". The completed form is to be printed out to bring to the lodgement.

• The guarantor must sign the guarantor page in the form and endorse of one of the child’s photos.

• Both parents must sign parental consent page in the form witnessed by a person who is not related by birth or marriage to the parents or child.

• Original Birth Certificate issued by Kantor Catatan Sipil or issued by the equivalent of the Civil Registry office. You would be required to supply an English translation of the document if the certificate's contents are not (all) written in English. A list of sworn translators that are listed with the Embassy can be found on the following link:

• Original Citizenship by Descent Certificate (CBD) issued by the Department of Immigration and Border Protection (DIBP) for those who were born outside of Australia.

• If your child was born in Australia, we will need to sight the original birth certificate issued by the Registrar of Birth, Deaths, and Marriages (RBDM) and one other relevant document. Please refer to section 3a of the PC8 form for further details.

• Original current passport.

• Two (2) photos (45 mm x 35 mm, chin to crown measurement is between 32-36mm with white/light blue background). One of the photos should be endorsed by the Guarantor.

• The lodging parent’s current and original passport.

• Original parent's marriage certificate, if current mother's surname is different from that in the birth certificate.

• If the child applicant is 10 years old or above, he/she is required to sign on the form

Passport validity will be 5 years.
Processing time at least six weeks.

Child Passport Application (16-17)

Please provide in the 'Comments' field:

* Your full name and date of birth
* Your address, telephone number and email address
* Your lost/stolen passport number (if applicable)

Please bring to the interview:

* Two (2) photos (45 mm x 35 mm, chin to crown measurement is between 32-36mm with white/light blue background). One of the photos should be endorsed by the Guarantor.

* An Indonesian Police Report in Indonesian (if passport was lost/ stolen). This report will also be needed later for you to arrange a new visa from the Indonesian Immigration.

* Completed form (Overseas Passport Application Form/PC8). The form can be completed on-line at www.passports.gov.au. On the main menu please click "Applying for an Adult Passport while Overseas".

* Completed B-11 Form (General Declaration for passport applicant) to explain your situation background on the reason why you need an Emergency Passport. (this form is provided to you at the Embassy).

* Any other photo IDs/medicare/credit cards that would support your identity and current residence.

* Flight itinerary to show your need of urgent travel.

NOTE:
This passport is intended for return to Australia only. If you are travelling other than Australia, please check with the Embassy of your destination country, as many countries does not accept entrance with an emergency passport.

Apostilles and Authentication

Please note that issuing an apostle / authentication of a document will take approximately 5 working days, as the signature on the document will need to be verified. You may be requested to contact the institution whom released the document if the signature of the person in the document is not in our database.

Certifying Copy of a Document

Certificate of No Impediment to Marry

* Complete a CNI application form here (which we will provide) and have your signature witnessed by us.

* We sight you and your fiancé’s original and current passports (if your fiancé do not have a valid passport, we will need to see his/her valid ID such as a KTP/SIM)

* We sight you and/ or your fiancé’s original divorce certificate (if any of you or your fiance have had previous marriage(s)

Appreciate fitting us in for the appointment when time zones caused confusion. Can I recommend when an appointment is booked it is shown in WIB time and not AEST time on the confirmation email? That would make more sense.
