Services
Passport Services
You may submit a maximum of two passport application under one appointment slot.
The passport renewal application form can be completed online from the APO on www.passports.gov.au (under Adult Passport Information, click on "Renewing an Australian Adult Passport") print the form and submit it to the Embassy. The requirements for an Adult Passport Renewal are:
• The completed passport renewal application form (PC7). Please ensure that the application which you have is that of a PC7 (2 pages in total, 1 cover page and another the application form itself) and that your signature on the application form stays within the provided white box.
• Your current original passport.
• Another current and photo ID, such as your KITAS/Driver's Licence as proof of current residence.
• Two (2) photos (45 mm x 35 mm, chin to crown measurement should be between 32-36mm with white/light blue background) are also required. Please refer to the photo guide attachment or see pp.2 of the application form for more detailed guidance.
• Processing time is at least six weeks after all requirements have been fulfilled correctly & payment received. Please note that priority processing is not available for applications lodged outside of Australia, London and Washington DC.
Passport validity is 10 years.

• If your child was born in Australia, we will need to sight the original birth certificate issued by the Registrar of Birth, Deaths, and Marriages (RBDM) and one other relevant document. Please refer to section 3a of the PC8 form for further details.
• Original current passport.
• Two (2) photos (45 mm x 35 mm, chin to crown measurement is between 32-36mm with white/light blue background). One of the photos should be endorsed by the Guarantor.
• The lodging parent’s current and original passport.
• Original parent's marriage certificate, if current mother's surname is different from that in the birth certificate.
• If the child applicant is 10 years old or above, he/she is required to sign on the form

* An Indonesian Police Report in Indonesian (if passport was lost/ stolen). This report will also be needed later for you to arrange a new visa from the Indonesian Immigration.
* Completed form (Overseas Passport Application Form/PC8). The form can be completed on-line at www.passports.gov.au. On the main menu please click "Applying for an Adult Passport while Overseas".
* Completed B-11 Form (General Declaration for passport applicant) to explain your situation background on the reason why you need an Emergency Passport. (this form is provided to you at the Embassy).
* Any other photo IDs/medicare/credit cards that would support your identity and current residence.
* Flight itinerary to show your need of urgent travel.
NOTE:
This passport is intended for return to Australia only. If you are travelling other than Australia, please check with the Embassy of your destination country, as many countries does not accept entrance with an emergency passport.

You may be requested to contact the institution whom released the document if the signature of the person in the document is not in our database.
